Hospital Discharge
Hospital Discharge
Hospital discharge marks the transition from hospital care to recovery at home or another care facility. It’s an important step that ensures you leave the hospital safely, fully informed, and prepared to continue your recovery with confidence.
Before discharge, your healthcare team will review your treatment progress, explain any new medications, and provide instructions for follow-up appointments, wound care, diet, or activity restrictions. They’ll also answer any questions you may have about your condition or recovery plan.
If additional care is needed — such as physical therapy, home nursing, or equipment support — your care coordinator will help arrange these services before you leave.
